Do you or someone you know live with a chronic health condition? Examples of chronic health conditions include arthritis, cancer, heart disease, high cholesterol, obesity, Lupus, Parkinson’s disease or diabetes.
If you or a loved on has a chronic health condition – you are not alone. According the Centers for Disease Control and Prevention in 2012 about half of all adults, 117 million, have one ore more chronic health conditions. One in four adults have two or more chronic health conditions. The good news is you can create and/or maintain a healthy and happy life with a chronic condition.
At The Family Health Centers, we realize that most of the time you spend managing your health is not spent in the doctors’ office. For this reason we are offering a new workshop to compliment the care you receive in our clinic. Living Healthy is an evidenced-based health promotion program developed by Stanford University (also known as Chronic Disease Self Management Program). This six-week workshop offers education, experience and support in the practice of self-management. The Family Health Centers’ Hominy Valley location will be offering this workshop in collaboration with Land of Sky Regional Council, Area Agency on Aging on Tuesdays, February 24th – March 30th, from 5:30 – 8:00 pm. There is a $30 suggested donation for the 6-week course – but no one will be turned away due to lack of ability to contribute.
The workshop addresses issues everyone living with an ongoing health condition faces and will not conflict with existing programs or treatments. Each participant will receive the book, Living a Healthy Life with Chronic Conditions, and a relaxation CD. The 6-week series supports participants in practicing valuable self-management skills such as making weekly action plans, problem solving, decision making, using your mind to manage your symptoms and more. Topics such as stress management, getting a better night’s sleep, physical activity and healthy eating will also be covered.
