Medical Records: All Authorizations for the release of Medical Records TO The Family Health Centers and Authorizations for the release of Medical Records FROM The Family Health Centers should be faxed to The Family Health Centers at Fax: (828) 253-4830. We will document and forward your medical records and requests to our medical records partner, Medicopy. Please do not fax medical records or requests directly to MediCopy.
We now offer Telehealth service:
It’s a virtual visit conducted with a provider over the telephone from a safe location of your choice. Telehealth supports the recommended social distancing that could help limit the spread of the COVID-19 virus. Please call (828) 258-8681, option 1, to request a Telehealth appointment.
A provider will contact you at the designated telephone number at the scheduled appointment time. Your co-pay and deductible will apply and will be billed at a later date.
