About this topic

An Advance Directive is a written statement of a person’s wishes regarding medical treatment, often accompanied by a living will, to help ensure a person’s wishes are carried out should the person be unable to communicate directly with a doctor or other caregiver.  We recommend that you submit completed Advance Directives for your records with The Family Health Centers.
