1. When and where will the sessions take place?
Group sessions will meet for 8 weeks on Mondays 5:30 PM to 6:30 PM starting March 3, 2025. The Whole Self challenge meets at 261 Asheland Avenue, Suite 103, on the upper level of the building.

2. What will the groups focus on each week? (subject to change)
• Week 1 – Welcome, introductions
• Week 2 – Whole foods, plant-based nutrition
• Week 3 – Exercise
• Week 4 – Healthy relationships
• Week 5 – Stress management
• Week 6 – Sleep
• Week 7 – Moderation
• Week 8 – Conclusion

3. What will it cost?
Each group session will be billed through your insurance and count towards your deductible. Your out-of-pocket cost will be about the same as your co-pay for a standard acute office visit. This will vary with insurance plans. If you have a high deductible plan, these groups will cost significantly more. Your co-pay for the first 4 classes will be collected prior to the first session. The copay for the last 4 classes will be collected halfway through the course.

4. What if I do not have insurance?
If you have health insurance, a claim will be filed on your behalf. The cash value of Whole Self Challenge is $400. Similar to participants with a co-pay, 50% will be collected prior to the first session, and 50% will be collected halfway through. The cash price is only available for participants without insurance. The class is only available to current patients of The Family Health Centers.

5. What if I miss a week?
If you miss a session in the first half, your payment will be credited towards the second half. If you miss a session in the second half, credit will be applied to your account at the Family Health Centers. This credit will go towards a pending balance or will be applied to a future visit.

6. What’s included?
Each week, you will get resources to improve your health based on the week’s focus. You will have support with creating measurable and obtainable goals and lifestyle changes. With successful lifestyle changes, providers may change medications as appropriate.
